* Assess and monitor patients' conditions and develop individualized care plans.

* Perform routine procedures and administer medications as prescribed.

* Maintain accurate documentation and confidentiality.
*Requirements:*
* Minimum 6 months of LTC experience.
* Current BLS certification.
* Valid NY RN license.
* COVID vaccination required.
