Qualifications:
- Master’s degree in Social Work from an accredited institution is required.
- At least one year of experience in a healthcare setting.
- Current Illinois Social Worker License (LSW or LCSW) or eligibility is necessary.
- Valid driver’s license
Skills & Knowledge:
- Strong understanding of social work methods and case management specific to Home Health.
- Proficiency in family assessment and adapting skills to diverse patient needs.
- Knowledge of public health administration principles is a plus.
Who You Are:
- Motivated to join a supportive environment recognized for its commitment to patient care and professional development.
- Able to establish effective working relationships with healthcare team members and adapt to various patient populations.
- Excellent communicator with the ability to handle complex social and health-related issues with empathy and expertise.
Work Schedule: Full-Time, Day Shift (Monday - Friday, 8:00 AM - 5:00 PM); Comprehensive benefits package including medical, dental, vision, free life, accidental death, and disability insurance.
Compensation: $30.29/hr to $39.42 per hour; W2 exempt
