Responsibilities

Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

  • Implement and monitor patient care plans. Monitor, record, and communicate patient condition as appropriate.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Note and carry out physician and nursing orders.
  • Assess and coordinate patient's discharge planning needs with members of the healthcare team.
Requirements

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.  American Heart Association or American Red Cross accepted.
  •  Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

 

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Additional Preferences

As a Registered Nurse with 6 West Renal Diabetic, you'll be instrumental in providing exceptional care to patients with renal and diabetic conditions on our 23 bed unit. Our department is committed to delivering comprehensive assessment, treatment, and education to individuals managing complex renal and diabetic health needs. You'll work closely with our interdisciplinary team to develop and implement personalized care plans aimed at optimizing patient outcomes and improving quality of life. From improving blood sugar levels of the patient to collaborating in the treatment plan of the dialysis patient, our unit offers diverse opportunities for professional growth and impact in the specialized field of renal diabetic care. 

 

  • Provide direct care for renal and diabetic patients
  • Support and Educate patients and families – procedure and plan of care 
  • Evaluate patient responses to treatments and interventions Monitor, record, and communicate with patient conditions as appropriate throughout their stay and recovery
