A community of passionate individuals dedicated to their work Your Role as Director of Case Management You will be responsible for: Overseeing the daily functions and staff management of the Case Management department. Managing the interdisciplinary plan of care and discharge planning process focused on optimal patient care outcomes. Acting as a patient and family advocate to ensure services are tailored to meet individual needs. Providing guidance and training to Case Managers and staff in managing caseloads and understanding regulations and operational standards. Evaluating operations to maintain high-quality service consistent with organizational standards. Building relationships with insurance companies, case management firms, and healthcare networks. Celebrating the achievements and contributions of your team along the way. Qualifications We are looking for candidates who possess: Current CCM® or ACMTM certification (preferred). The ability to independently complete assessments within their professional scope. An active license if required for their discipline. A Bachelor's degree in nursing (BSN) for nursing candidates, or at least a Bachelor's degree in another applicable healthcare field (a graduate degree is preferred). A minimum of three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning. The availability to work on weekdays, weekends, evenings, or nights as necessary. The flexibility to work on religious and legal holidays, as scheduled.
