Boulder Community Health is seeking an Occupational Therapist who assumes responsibility and accountability for providing quality care to patients with disorders affecting daily living skills, sensory-motor functions and cognitive/perceptual functions.

Qualifications
•Current registration as an OT with the State of Colorado
•BLS certification required by the end of conditional period
•Experience in an inpatient rehabilitation environment with an interdisciplinary team is preferred.
•New Grads considered, must have had a Level 2 Rotation in Acute Care.

Responsibilities
•Evaluative skills: selects appropriate tests and measures to determine goals and problems, course of treatment and appropriate therapeutic interventions.
•Assessment skills: identifies problem list/ assessment results, evaluates patient response to treatment and revises plan of care as indicated.
•Planning skills: formulates and maintains an individualized treatment plan based on functional long and short-term goals.
•Implementation skills: provides treatment using appropriate techniques and tools.
•Promotes effective communication and teamwork.
•Reports and documents evaluations, plans, treatment techniques, assessment and patient progress.
•Initiates and is responsible for problem solving and decision-making regarding patient care.
•Functions as an advocate for the patient within the rehabilitation system. Identifies patient/family goals and incorporates this feedback into the overall plan of care.
•Transfers patients from wheelchair, bed, mat, commode etc.
•Transports patients to/from their room to the therapy room.
