And we support our partners with scalable solutions to meet the health care needs and costs of aging populations.The Growth team focuses on amplifying the impact of the PACE model of care by educating and expanding access to the program. Our team manages the end-to-end process of identifying, attracting, engaging with, assessing, and enrolling new participants. The Director, Growth Strategy & Operations will support the development and implementation of strategic initiatives in support of the team's mission.Core Responsibilities & Expectations for the RoleDrive strategic growth initiatives (comprehensively from analyses & research, recommendations, stakeholder alignment, operational alignment, to rollout) across activities related to expanding the participant population, such as:New market strategyTop-of-funnel referral generationOperational improvements across the enrollment journeyCall center strategy & operationsProgram design inputSeamless handover to care teamsDesign of new operational processesSupport partner management (e.g., Kaiser Permanente, our JV partner in California) including owning key relationships, building collaborative tools and workflows to educate/enable clinicians and refer patients who may benefit from PACE, and problem solving operational challengesSupport development and management of key Growth platforms and tools (including Salesforce and EPIC)Support regular reporting and performance tracking, monitor success, elicit feedback and input on initiatives, and identify opportunities for / drive continuous iteration and improvementCollaborate across the organization, including supporting key strategic decisions and building assets related to clinical operations, participant experience and journey, health plan operations, and product development as they intersect with growth prioritiesFacilitate cross-functional & external stakeholder engagement to align on and drive progress against key initiativesQualifications:5+ years of relevant experience, such as in health care delivery at a high-growth startup or consulting for health care payers & providersDemonstrated ability to be a self-starter, with ability to excel in a fast-paced, unstructured work environment, comfort operating in ambiguity, and a bias to actionExcellent analytical and project management skillsStrong interpersonal and written/oral communication skills; high EQA bachelor's degree is required.Strong problem solver who can connect high-level strategy to daily executionThrives in fast-paced, evolving environments and builds structure where none existsDeep cultural alignment with Habitat's values and a strong passion for our missionWillingness to work in a hybrid work environment, with travel to Habitat Health centers as neededProof of medical clearance for communicable diseases, including a TB test.Proof of all immunizations are up to date.Proof of valid CA driver's license, personal transportation, good driving record and auto insurance as required by State law.Compensation:We take into account an individual's qualifications, skillset, and experience to determine final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected base salary range for this position is $161,000 - $173,000 and is bonus eligible.
