Your Role as Home Health LPN: Provide in-home care to patients recovering from illness, injury, or chronic conditions, under the supervision of an RN. Collaborate with a dedicated team of healthcare professionals to develop and implement quality patient care. Conduct patient assessments, monitor vitals, document progress, and communicate any changes to the RN supervisor. Assist in executing personalized care plans and educate patients and families on self-care techniques. Work alongside an interdisciplinary team to ensure seamless patient discharge and continuity of care. Qualifications: Graduation from an accredited practical nursing program and possession of an active LPN license in Colorado. A minimum of six months of LPN experience is required, with home healthcare experience preferred. Wound care and Med Surg experience is a plus. Current CPR Certification is necessary. Familiarity with state and federal home health regulations. Strong communication skills, compassion, and clinical proficiency will set you apart. Why Interim HealthCare?
