Risk ManagerAs a key component of Humboldt Park Health's Performance Improvement (PI) and Peer Review processes, the Director of Risk Management must coordinate all risk-related activities, communicate effectively and provide for the resolution of risk factors and events.Essential Duties and Responsibilities:Develops and implements an occurrence reporting system.Coordinates the CHRPP (Chicago Hospital Risk Pooling Program) at NAH.Will implement and monitor CHRPP occurrence report forms in all departments.Conducts training sessions on occurrence reporting for all employees.Investigates all occurrences that could lead to serious financial loss, includingreviews of the medical record, interviews of those who rendered care to thepatients/visitor (as appropriate) to get his/her account of the occurrence,interviewing witnesses (as appropriate).Provides a safe and confidential repository for all data relating to occurrencereports.Report to CHRPP on matters of possible serious exposure.Organizes and analyzes aggregate data from occurrence reports in order toidentify any document patterns of actual or potential problems in patient care.Reports aggregate occurrence-related data to the Performance (Quality) Improvement committee to identify areas of risk throughout the hospital.Recommends appropriate and timely action to hospital departments in response tooccurrence reports, patient complaints, and other sources of information.Works with appropriate personnel in acting on identified problems.Follow-up to ensure corrective action is taken and continued.Develops a mechanism for the documentation of identified problems and thetracking of corrective action and follow-up.Meets regularly and, as needed, with CHRPP staff.
