Family Health Services is a Federally Qualified Health Care Center located in Sandusky and Norwalk, Ohio, serving the Erie and Huron County area. We are currently looking for a Quality Director. The Quality Director will be responsible for leading the organization's Quality programs ensuring compliance with HRSA regulations. This position functions as the leader of all quality projects throughout Family Health Services, including primary care, dental, behavioral health, and any future service lines that are added. The Quality Director will build the quality program and oversee the evaluation of programs and services including UDS, Patient Centered Medical Home, and other outcomes. The director has the responsibility to ensure the highest quality services are provided to patients throughout the organization. They will plan, lead, and direct process and overall quality improvement activities that produce better patient care and more efficient operations.

Essential Functions
  1. Provide overall direction, coordination, and implementation of the quality programs at Family Health Services.
  2. Implement strategies and plans for quality improvement within service lines in collaboration with Directors and providers including monitoring and presenting internal quality measures, initiatives, and plans.
  3. Oversee CQI audits on patient charts to ensure best practices are being met.
  4. Coordinate data collection and submission for HRSA required reporting, including the annual UDS report.
  5. Participate in and assist in preparation for external audits and site visits by accreditation or regulatory agencies including HRSA, 340B, etc.
  6. Oversee development and implementation of all requirements for Patient Centered Medical Home (PCMH) certification and Chronic Care Management (CCM).
  7. Monitor FTCA application and compliance.
  8. Develop reporting through eCW/Azara to accurately pull data to analyze for PDSA studies and as indicated for operations, grants, etc.
  9. Analyze data collected through patient satisfaction surveys to identify areas of concern and provide recommendations for improvement to appropriate clinic management and staff.
  10. Oversee Manage Care Performance contracts and Panel Management.
  11. Develop methods for data collection and extract data as required and prepare QI reports, corrective action plans, and statistics.
  12. Lead the QI committee and be a part of the Risk Committee.
  13. Other quality and evaluation duties as assigned.
Competencies
  1. Must be familiar with PCMH accreditation.
  2. In-depth knowledge of Continuous Quality Improvement.
  3. Understanding of professional and legal/license-based practice standards in multiple disciplines relevant to patient care.
  4. Skill in planning, organizing, and problem-solving.
  5. Ability to communicate effectively with medical providers and staff.
  6. Excellent oral and written communication skills.
  7. Experience working with disadvantaged populations and knowledge of health disparities.
  8. Outstanding skills in data collection, analysis, and presentation.
  9. Skill in establishing and maintaining effective working relationships with patients, clinical staff, and the public.
  10. Ability to prepare and maintain records, write, and present reports.
Supervisory Responsibilities

Direct supervision of Patient Centered Medical Home Coordinator and Quality Panel Specialist.

Required Education and Experience

Bachelor's Degree in Nursing or Healthcare Administration, Health Information Management, or related field. 2-3 years of managerial supervisory experience. Experience in Quality Improvement in a healthcare facility, preferably with a federally qualified health center.

Preferred Education and Experience

Master's Degree in Nursing or Public Health. 2-3 years related Compliance/Quality Improvement experience in health care.
