Role Description
This is a full-time hybrid role for a Registered Nurse at Dignity Home Health and Hospice located in Layton, UT providing care for patients in Davis County. The Registered Nurse will be responsible for:
- Coordinate the overall care of the patient while interacting with patient, family, physicians, Interdisciplinary Team members and other healthcare professionals.
- Provide professional nursing care for both home health and hospice patients in their place of residence.
- Complete initial, comprehensive and ongoing assessments for patients to determine their needs.
- Conduct nursing visits for pain and symptom management, wound care, medication administration and other skilled care as ordered by the physician.
- Complete all documentation and charting accurately and in a timely manner.
- Monitor and supervise those involved ensuring that the patient receives the best possible care.
Qualifications
- Active Registered Nurse (RN) license
- Experience in providing direct patient care
- Ability to assess patients' conditions and develop care plans
- Knowledge of medication administration and monitoring vital signs
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize tasks
- Attention to detail and ability to maintain accurate patient records
- Ability to work independently and as part of a team
- Experience in home health or hospice care is a plus
- Bachelor's degree in Nursing or related field
