Overview

  • Healthy Beginnings is a long-term home visiting service, providing weekly home visits to low-income, expectant mothers and their families, during pregnancy and until the child associated with the pregnancy turns 2 years of age (up to 5 years if child is born with opioids in system). The team is comprised of a dynamic and passionate group of professionals who are equipped with specialized skills to provide care and support to the entire family. The team focuses on family health (pregnancy, post-partum, early childhood wellness, chronic condition identification and management), early childhood development, family dynamics, violence reduction, adult educational advancement and employment, social determinants of health, and truly anything a family may need. The team is dedicated to working with families to connect with our community network and the best healthcare resources available. Community Ambassadors are integral members of the Healthy Beginnings team. They serve as coaches, develop rapport with, and act as a liaison between families and healthcare providers. They have a key role in connecting people to social services, promoting family health and wellness, helping with health system navigation, and providing education and support. They assist the Early Childhood Educator by conducting developmental screenings/assessments and reinforce recommended educational plans when working with families. In addition, Community Ambassadors conduct educational classes to help meet the needs of families.

    EXPERIENCE REQUIREMENTS
    Must be enrolled in or a graduate of Healthy Beginnings Services. Previous public contact and proficient in basic computer skills. Other experience will be taken into consideration.

Interpersonal skills necessary for establishing and maintaining good working relationships with the client, family and interdisciplinary team. Creates a culture of caring, trust, as well as support and growth with families, community members, and team members.

    Essential Functions:
    • Conduct Home Visits for families enrolled in the Nurse home visiting program. Documentation of home visit encounter and activities/assessments performed.
    • Make recommendations for further screenings
    • Coordinate with Nurse Home Visitors on home visits and services provided for clients.
    • Assist clients in utilizing resources, including but not limited to scheduling appointments and application completion for community resource programs.
    • Assist with educational opportunities for adults.
    • Engage potential clients in recruitment discussions.
    • Assist in data collection, such as health outcomes, long term metrics, and early childhood.
    • Develop an overall knowledge of community and school resources, along with services and programs related to the needs of our clients.
    • Report suspicion of child abuse and/or neglect, as a mandated reporter, to the Department of Children and Family Services
    • Develop a deep understanding of all services offered through Community Health Initiatives in order to connect families and community members to appropriate services.
    • Demonstrate a deep understanding and respect for the community
    • Serve as the bridge between the community and Community Health Initiatives.
    • Advocate as the frontline agent of change, helping to reduce disparities in underserved communities.
    • Reduce barriers to care by addressing psychosocial needs and providing education, resources, support and navigation.
    • Act as a cultural broker to provide feedback to staff and patients on cultural issues that may affect patient‘s health, including was to address health disparities.
    • Actively participates in Case Conferences and Team Meetings for the Nurse Home Visiting Teams.
    • Assist and support early childhood services for the CHI division.
    • Assist Early Childhood Program Coordinator with Monthly Mom events and other Educational Outreach events including, but not limited to Home Visiting social events.
    • Perform health screenings, health education, and other medical assistant duties
