Bridges Palliative Care is seeking a part time (3 day/week) certified registered nurse practitioner to provide palliative care services nursing facilities located in Chester & Berks Counties. This Palliative Care Nurse Practitioner will provide expert consultative services to patients with advanced disease who require palliative services through disease management and support. The CRNP will work collaboratively with the patient's physician, nurses, specialists and social workers to provide coordination of care to effectively meet the needs of the patient while allowing the patient to provide input on their care needs and wants.
CRNP will be responsible for assessment of patient, identifying service and support needs through both clinical assessment and care conversations including end of life decisions. Other responsibilities include completion of POLST forms, review and ordering of medications, labs and other diagnostic testing and communicating with patient's in-house care team. Ideal candidate will have an interest in or experience in palliative care services. Experience is a plus but not a requirement. This is a part time role, responsibilities are in house in skilled nursing facilities 3 days a week and on-call via phone until 8 PM each night.
