As a Post Acute Care Specialist in our Inpatient Rehabilitation Hospital, you will play a vital role in ensuring our patients receive high-quality, efficient care. We are a dedicated team committed to excellence in patient care. If you share our passion, we invite you to apply Job Summary and Qualifications In this role, you will utilize various patient information sources and collaborate with other members of the patient's care team to identify individuals who may benefit from acute inpatient rehabilitation or long-term acute care hospital (LTACH) services prior to their return home. You will review these patients daily, working closely with the assigned case manager until there is sufficient clinical evidence to request a formal pre-admission assessment. Qualifications Required: Education: Graduation from an accredited school in a clinical health care specialty (RN, Physical Therapist, Occupational Therapist, or Speech Therapist). Licensure: Current state licensure in your clinical practice area or eligibility for state license transfer. Experience: Minimum of 2 years of recent clinical care experience. Skills: Strong verbal and written communication skills, organizational abilities, proficiency with computers, and effective problem-solving skills. Certification: Must obtain relevant clinical licensure within 30 days of employment. Degree Requirements: Bachelor’s, Master’s, or Doctorate Degree in a relevant field.
