Case Manager (Bilingual in English and Mandarin)

Position Summary:
The Case Manager assesses clients needs and eligibility for in-home services, entitlements, and benefits, while overseeing ongoing case management and care plan implementation.

Principal Responsibilities:

  • Conduct client intakes and screenings when necessary.
  • Provide accurate and pertinent information on benefits, entitlements, programs, and case assistance, aiding clients in accessing services through referral, counseling, and advocacy with external agencies.
  • Facilitate the application and recertification process for clients' entitlements and benefits.
  • Coordinate the provision of services as required.
  • Communicate respectfully and empathetically with clients, families, and stakeholders.
  • Utilize the program's data system proficiently to maintain comprehensive and precise case notes, monitor reassessment, client data and contacts, service delivery, and financial information.
  • Prepare and maintain all essential documentation in client case files.
  • Generate statistics, compile reports, and disseminate information as needed.
  • Actively engage in supervision sessions and staff meetings.
  • Complete assigned tasks thoroughly, accurately, and within organizational deadlines.
  • Assess clients' social, emotional, and environmental status in their respective settings.
  • Develop, implement, and monitor care plans for clients.
  • Maintain ongoing contact and provide supportive counseling to clients.
  • Provide supervision to social work students as required.
  • Arrange translation services for clients as necessary.

Qualifications 

  • Bachelors degree is required. 
  • One year related work experience and/or training
