Client location and schedule hours:
Female client in Brentwood. Schedule is onday through Thursday 12pm  to 5 pm

Job Responsibilities:

Social History:  Female client , widow. No pets.  She is still very independent & loves to take a 1 mile walk each morning. She enjoys grocery shopping, watching TV, talking to her brother or friends and listening to her records.

Diagnoses:  Short term memory loss, Sundowner's every afternoon, Glaucoma (Slight vision loss) and bladder urgency.                     Allergies:   None to food or medicine

Goals and Objectives:  Help prepare/serve meals, companionship, driving to appts or errands. Hydration reminders, light housekeeping (vacuum, clean kitchen, change bed linens every other week. Remind to get shower every other day- ask if she needs help, or change to fresh clothes daily. Encourage to brush teeth daily. Take walks with her, help her with her cell phone, help her organize areas of her home. Maintain running grocery list.

ADL's:    Mobility:  Walks independently. No cane or walker needed.   

Eating:  Client feeds herself without assistance. She needs meals/snacks suggested and served to her.   

Toileting:  Client has bladder incontinence and wears a sanitary pad most of the time.  She toilets independently. 

Bathing/Dressing/Grooming:  Client bathes independently. She has a shower chair. She dresses herself but needs to be reminded to put on clean clothes each day. She needs to be reminded or get help to brush her teeth. 

Communication:  Client speaks clearly and can make her needs known. She is articulate, engages well and does not have a hearing loss. She does repeat herself a little but will answer questions. 

Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language 
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
TB test
