EMR Virtual training - Only able to attend if Modules are completed before training. Virtual EMR training must be scheduled prior to the providers first day, to avoid the provider missing their first day. Travel Preferences: Preferred Lodging: Preferred hotels provided - Reach out to Trio Operations to obtain the list of preferred hotels with a corporate rate. Air Travel: Local/drive-in candidates only - Highly preferred. Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Patient population, Adolescent, Adult Number of beds, 8 Admissions: 1 per day Do they have in-house anesthesia? Yes Number of deliveries per month? 30 Is this covering an OB/GYN practice, backing up family practice or midwives? Yes for family practice, no midwives Site is unable to schedule providers more than 4 consecutive days for patient safety. Credentialing Information: 60 days temps / 120 days full Credentialing with other facilities? Yes Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less?: No Requirements : Active Unrestricted CO License or IMLC (LOQ at time of submission) Board Certified OB/GYN - Required Active DEA - Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)
