What You'll Do: As a key member of our interdisciplinary care team, you'll support patients and families by providing emotional guidance, resource coordination, and advocacy in the home health and hospice settings. You'll collaborate closely with nurses, therapists, and physicians to ensure comprehensive care and a strong support system for every patient.
