Home Care Responsibilities:
- Provides care as defined within the scope of the state’s nurse practice act
- Conducts non-medical in-home assessments/re-assessments and creates Plan of Care and Task Lists for caregivers
- Client Relations – This includes, client care needs and family concerns
- Under the direction of the Director of Nursing, working together with Client Care Supervisor and Customer Care Manager facilitates the home care client process is smooth (inquiry to discharge)
- Assures proper maintenance of clinical records in compliance with local, state, and federal laws
- Participates in agency performance improvement activities, and continuously staying informed about changes in the field of nursing and home health care
- Compliance/Promotion of the standards of quality set forth by BrightStar Care.
- Thorough knowledge of the regulations/accreditation processes to ensure compliance with all Joint Commission, regulatory, HIPPA, OSHA and local, state, and federal laws.
- Ensure high-quality customer care is provided at every stage of the client/caregiver touch-points; phone to in-home consultation
- Assists with competency of new field staff
- Supervises, educates and coaches caregiving staff
- Assures proper maintenance of clinical records in compliance with local, state and federal laws
- Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel
- Promotes home care referrals in the health care community
- The nurse will provide direct care to clients, as needed
- Makes initial client assessment visits and regular reassessments of the client’s nursing needs, as needed
- Additional assignments as needed. Perform supervisory visits
- Uphold BrightStar Care behavior standards
- Assist with TB tests and influenza shots
- Assist with skilled IV hydration therapy
Requirements:
- RN with one (1) year clinical experience, and some office experience preferred
- Current state license to practice in state care is being provided
- Preferred: Bachelor’s degree in Nursing. Must have graduated from a National League of Nursing accredited program.
- Comply with accepted professional standards of practice.
- Analytic/Critical thinker.
- Possess exceptional nursing assessment skills
- Comply with accepted professional standards of practice.
- Excellent interpersonal skills.
- Excellent customer service skills and ability to deal tactfully with clients, staff and community.
- Treat clients, staff and the public with courtesy, respect and presents a positive public image.
- Case management skills are a plus
- Reliable transportation
- Registration on the Home Care Aide Registry - two weeks after hire
