Role Description

This is a full-time on-site role as a Case Manager at OneWell Health Care in Lancaster, PA. The Case Manager will be responsible for assessing the needs of individuals, developing and implementing care plans, coordinating services and resources, monitoring progress, and advocating for individuals and their families. The Case Manager will also provide guidance and support to the care team and collaborate with community partners to ensure the best possible outcomes for the individuals we serve.


Qualifications

  • Strong communication and interpersonal skills
  • Ability to assess and evaluate the needs of individuals
  • Experience in developing and implementing personalized care plans
  • Knowledge of community resources and services
  • Ability to collaborate and work effectively with interdisciplinary care teams
  • Excellent organizational and time management skills
  • Empathy and compassion for individuals with Intellectual and Developmental Disabilities
  • RN (Registered Nurse) License (required)
  • Bachelor's degree in Nursing (preferred)
  • Experience in Case Management or a related field (preferred)
