Key Responsibilities:

  • Coordinate and facilitate patient care progression throughout the continuum, working collaboratively with physicians, nursing staff, and other members of the healthcare team.
  • Ensure effective utilization of resources, manage the length of stay, and support cost-efficiency without compromising quality of care.
  • Develop and execute individualized care plans that address patient needs across various stages, including post-acute care.
  • Engage in discharge planning and execute timely interventions to streamline patient transitions between care settings.
  • Implement and monitor care management practices that contribute to improved patient outcomes.
  • Maintain compliance with healthcare regulations and standards while advocating for patient needs and optimal care delivery.


Requirements:

  • Bachelor’s Degree in Nursing preferred; Master of Social Work accepted.
  • Valid Texas RN or LMSW license required.
  • Certification in Case Management (obtained or to be obtained within two years of employment).
  • Minimum of three years of experience in an acute care hospital setting or similar clinical environment.
  • Proficiency in utilization management, discharge planning, and resource management.
  • Exceptional skills in communication, negotiation, and interdisciplinary collaboration.
