Community Based Access/Discharge Team Clinician

Pay: $72,028.06 annually This position may be eligible for a sign on bonus of $5,000 after 60 days of employment if selected applicant meets all policy conditions.
Work Schedule: Monday-Saturday, between 7am-7pm, 40 hours, with some evening hours based on patient needs
Provide goal-directed, youth-centered, medically necessary individual, group, and family therapy to adults and youth with mental health and/or addictions as part of the Certified Behavioral Health Clinic's Children's Services Access and Discharge team. The employee will work directly with youth and their families to provide short-term therapeutic support, crisis intervention, and stabilization services to assist youth in transitioning successfully from acute psychiatric hospitalization, Psychiatric Rehabilitation Treatment Facilities (PRTF), and higher levels of care back to their homes and communities.
Service Provision Therapy and Evaluation Services
The intake/assessment will include history, presenting problems, and diagnoses. Based on youth and caregiver availability, intake assessments may occur in person in acute psychiatric hospital settings, in the youth's living environment, or via telehealth.
Complete Child and Adolescent Functional Assessment (CAFAS) and assess youth for the HCBS SED Waiver.
Conduct risk assessments as needed to address the safety of the youth.
Conduct family therapy sessions in the home environment, addressing family dynamics, communication, and conflict resolution as indicated.
Work collaboratively with parents, caregivers, and siblings to strengthen family relationships and improve overall functioning.
Assist families in implementing therapeutic strategies to enhance stability and reduce risk factors.
Provide evidence-based, time-limited individual therapy to youth, focusing on trauma-informed care, crisis stabilization, and skill-building.
Respond promptly to crises, providing immediate support and safety planning.
Collaborate with emergency services, schools, and other community resources during crisis interventions.
Develop crisis prevention plans tailored to each youth's unique needs.
Treatment Planning
Prepare quality treatment plans in collaboration with youth and caregivers, additional treatment team members, and community partners.
Facilitate treatment plan review, addressing updates and progress on goals with youth, caregivers, treatment plan coordinator, and other treatment team members.
Collaboration and Transition Support:
Work closely with multidisciplinary teams, including but not limited to psychiatrists, case managers, and educators.
Advocate for youth and families within the community, ensuring their needs are met.
Attend meetings, court hearings, and other relevant appointments as needed.
Coordinate with acute psychiatric hospital clinicians, PRTF clinicians, outpatient providers, and school personnel to ensure continuity of care of therapy services post-discharge or diversion from higher levels of care.
Minimum Qualifications: One year of experience providing therapeutic services to youth and/or adults. Master's Degree or higher. Must be licensed by the Behavioral Sciences Regulatory Board as an LMSW, LMFT, LMLP, LPC, or LP. Employees hired with a temporary license must test for permanent licensure within 90 days of hire. Per Sedgwick County policy, this is a driving level position that requires a valid US driver's license without restrictions and current proof of automobile insurance. Must have access to a personal passenger vehicle to complete the driving duties of the position. Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens. Must complete orientations provided by Sedgwick County and COMCARE.
Preferred Qualifications: May be clinically licensed by the Behavioral Sciences Regulatory Board as an LCP or LCPC, LCMFT, or LSCSW.
