Medigrace Travel Nursing has private duty assignments in middle Tennessee that will allow you to have more personal time and avoid the commute.
We are looking for Registered Nurses and Licensed Practical Nurses to provide home health care in Pleasantville, Hartsville, Portland, Lafayette, Lavergne, Gallatin, Brentwood, Franklin, Nashville, Lebanon, and Cottontown.
Schedule:
- 8 or 12 hour shift
- Days, evenings, and nights available
- Part-time and Full-time available

About the Job
Responsibilities:
- Perform various activities associated with daily living.
- Care for patients one-on-one in the patient’s home to provide a comfortable environment.
- Perform other personal care services as necessary to meet the patient’s needs.
- By following these guidelines, home health nurses ensure that patients receive comprehensive, compliant, and compassionate care in their homes as prescribed by the physician.
- Conduct patient assessments and coordinate care.
- Monitor vitals and gastrointestinal (GI) intake and output.
- Assess skin integrity and administer wound care.
- Monitor for changes in the patient’s condition, such as weight loss/gain, self-care abilities, and indicators of disease progression.
- Follow compliance guidelines throughout all processes, including OSHA, FDA, and HIPAA regulations.
- Complete compliant documentation of the care provided in the system of record.
- Manage and administer prescribed medications, treatments, and therapies as needed
- Clearly communicate with family members, on-site coordinator, physicians, case managers, and other prescribed individuals regarding the health of the patients when needed
- Experienced in providing basic medical management up to care for medically fragile patients at the end of life.
Qualifications:
- Current State or Compact License as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- Current CPR, ACLS, and BCLS Certification
- Completed TB test
- Pass Background Check & Drug Screening
Skills and Abilities:
- Excellent interpersonal skills.
- Ability to stand or walk for extended periods, with the capability to lift up to 50 lbs and assist in moving patients.
- Capability to operate effectively under stressful conditions and make quick decisions.
- Dependable transportation and willingness to commute to patients' homes.
