RN Home Health Case Manager - Milwaukee North (West Allis, WI) COVID Rate | 40 hrs/week | Days | Jan 12-Apr 11, 2026 If you're a home-health-savvy RN who's all about strong assessments, independent workflow, and real continuity of care, this contract is a solid fit. Straight weekday schedule, no call, no weekends-very classic HH setup. Shift & Schedule Monday-Friday 8:00 a.m.-4:30 p.m. 40 hours/week No call No weekends Holidays as needed May float/assist other HH teams Coverage Area (North Milwaukee Zone) 53097, 53092, 53223, 53217, 53209, 53224, 53218 Required Experience Minimum 1 year Home Health RN experience Must be confident managing: OASIS documentation Start of Care visits (target: ~3 SOCs/week) Patient teaching and independent care planning Strong computer documentation skills Valid WI or compact RN license Active BLS (AHA) Clinical Skills Needed Wound care: wound vacs, pleural drains, ostomy care Peripheral blood draws Central line care IV administration (elastomeric devices, IV pumps) High-level assessment and triage judgment Ability to work independently in the field Other Requirements Reliable transportation Valid driver's license Current auto insurance Orientation Standard onboarding through the Home Health team Additional support available through other teams if floating occurs
