Role Overview:

The Registered Nurse (RN) will deliver personalized care and services, attending to a designated patient group in various settings like private residences, Nursing Facilities, and assisted living centers. Collaborating within an interdisciplinary team, the RN will offer expert guidance on psychosocial assessments, interventions, and community resources, taking on roles in Case Management, Admissions, or Resource allocation.

Required Qualifications:
  • Active Registered Nurse (RN) license in the state(s) of practice
  • Ability to work effectively in a team setting, coordinating with nurses, social workers, chaplains, volunteers, and CNA leadership daily
  • Experience in providing personalized care and receiving support from a patient care team
  • Proficiency in independent work, handling daily tasks like communication, meetings with healthcare professionals, and adherence to hospice regulations

Strong time management skills for organizing visits, documentation, and meetings are crucial, with the ability to prioritize tasks and address urgent needs promptly. An understanding of electronic medical records systems is preferred. Candidates must have reliable transportation, a valid driver's license, and auto insurance for travel within the service area.
