Overview:

We are seeking a Physical Therapist for a growing organization in Illinois to provide outpatient therapy in home-based settings within the Oak Lawn, IL area. This position can be full-time, part-time, or PRN, with a minimum requirement of 12 hours per week and 3 weekdays of availability. The Physical Therapist will primarily work with the geriatric population and provide community-based outpatient services billed as Medicare Part B.

Job Responsibilities:

• Deliver one-on-one direct patient care in patients' homes.
• Work within a multidisciplinary therapy team to treat a balanced mix of patients.
• Develop treatment plans tailored to each patients needs in their home environment.

Compensation:

This salaried position includes a generous benefits package and opportunities for overtime pay.

Job Requirements:

• Graduation as a Doctor of Physical Therapy (or equivalent standing).
• Eligibility for Illinois state licensure as a Physical Therapist.
• Availability to work a minimum of 12 hours per week.

• PRN/Per Diem candidates will perform ongoing one-on-one direct patient care.
• Regular and consistent weekly availability is required, including 3 weekdays.

• New graduates will receive support and mentorship for full-time positions.
• Some prior work experience is required for PRN/Per Diem and part-time candidates.
