Join us and make a lasting impactHow you will contribute (not limited to):The Clinical Manager is accountable for overseeing the day-to-day clinical operations to ensure quality of care & services, regulatory compliance, support of business development & growth, achievement of key performance indicators, and quality outcomes. Responsible for the oversight, direct supervision and development of clinicians and direct-care staff.Location: GJ BranchCoverage area: Mesa & Garfield CountiesSchedule: Mon - Fri, Admin on call rotationOur comprehensive benefits include:Competitive compensationMileage reimbursementEmployee referral program401(k) w/ company matchEmployee discountsTuition reimbursementPaid time offMedical, dental, and visionHealth Savings Account (HSA)Flexible Spending Account (FSA)ResponsibilitiesConducts/participates in quarterly quality improvement meetings, reviews pertinent available data, identifies trends and oversees the implementation of improvement plansCollaborates with Branch Director in the implementation of quality improvement activities and corrective action plansProvides feedback to Operations management regarding necessary changes and revisions relating to care provisionAccountable to ensure all clinical staff appropriately and successfully complete orientation and all required in-servicesResponsible for the oversight of and response to external and internal surveys and audits within assigned area relating to the quality of care and clinically related KPIs in collaboration with operations leadershipActs as a resource and has a working knowledge of payer requirements relating to the provision of care and documentation guidelinesCompletes documentation and chart reviews to identify variances in standardized care and care processes and to identify areas for improved performance related to patient care and the organizationHolds case conferences to ensure oversight of care, coordination of services and that standards are metEnsures documentation standards are met per company standards and to support quality standards, coverage, and reimbursementEnsures that care is coordinated with all involved in patient careIdentifies variances in standardized care and care processes to identify areas for improved performance related to patient careEnsures compliance with regulatory, payer and company requirementsFacilitates the development of performance improvement action plans, based on available data (e.g., survey results, QAPI and clinical record reviews) Accomplishes staff results by communicating job expectations; planning, monitoring, and appraising job results; coaching, counseling, and disciplining employees; developing, coordinating, and enforcing systems, policies, procedures, and productivity standardsOversees and directs the selection and hiring of clinical staffConducts interviews and makes hiring decisionsEnsures staff competency and performanceEnsures assigned staff are evaluated fairly and timely in accordance with company performance evaluation proceduresEnsures direct, on-site supervision for all staff per company requirementsResponsible for achieving organizational performance and retention goals among clinical staffServes as a resource in assisting the field staff with problem solving to provide the most appropriate care for the patientServes as a coach, mentor, and role model to all professional and direct-care staffEnsures adequate numbers of preceptor staff to provide development and mentoring of staff clinicians and direct-care staffAt least one preceptor clinician is trained to provide back up for the clinical manager positionOther duties as assigned may be asked to perform patient visits and perform on-call/after-hours duties in the event of staffing issuesQualificationsBachelor's Degree preferred in Nursing or Physical Therapy from an accredited school of nursing or therapyLicensed Registered Nurse (RN) or Physical Therapist (PT) in good standing and currently licensed by the StateLess than two years clinical experience, and at least one year in related management/supervisory role in home health careCurrent CPR certificationMust meet all agency requirements for pre-employment as required by the company and/or State regulationsAbility to use company documentation systemsAbility to communicate (verbally and written) with all levels of personnel, internal and external to the companyAbility to work independently as well as part of a teamCapable of working responsibly with highly confidentialAbout Our Line of BusinessAt Abode Hospice and Home Health, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
