Hospice of Orange and Sullivan Counties is seeking a full-time Nurse Practitioner to join our team.As a member of the interdisciplinary team, the Nurse Practitioner will provide meaningful, end-of-life care where it matters most-at home, in nursing facilities, and at our Kaplan Family Hospice Residence. You'll be joining a dynamic group of NPs and physicians in a collaborative environment where your voice matters. The work of a hospice Nurse Practitioner is impactful and gratifying. This is the kind of medicine you've always dreamed of practicing. Practice with purpose. Work with heart.Schedule: M-F, 8:30am to 4:30pm. One (1) weekend on call per month. One (1) to two (2) holidays on-call per year.Responsibilities may include: Conducting in-home patient visits for symptom management, collaboration of care, goals of care discussion, assessment of complex wounds, and evaluation of eligibility for hospice benefits. In collaboration with the medical director and intake nursing staff, reviewing clinical documents to determine eligibility and terminal diagnosis. Serving as the agency's on-call provider, which includes advising nursing staff on symptom management, prescribing medications including controlled substances, and completing death certificates. Serving as a clinical expert to nursing and other staff. Participating in community education and outreach. RequirementsEducation, Skills & Experience: MS Nurse Practitioner or DNP Doctorate Nurse Practitioner with NYS license and national certification. Must hold a current NY State RN license. DEA license to prescribe controlled substances in New York State. Current State of New York driver's license and automobile insurance. Two (2) years of Nurse Practitioner experience, preferably in primary care or home health. Strong preference will be given to candidates with hospice experience and certification.
