BHN is currently seeking a compassionate and motivated Case Manager (BA Level) to join our School-Based Community Service Agency (CSA) team in Springfield, MA. What We Offer Comprehensive benefits including health, dental, vision, and retirement match Generous paid time off and holidays Career advancement opportunities with one in three positions filled internally Ongoing training, supervision, and professional development A supportive, collaborative, and mission-driven work environment Recognition as both a Certified Great Place to Work and one of Massachusetts' Top 10 Employers What You'll Do As a Case Manager (BA Level), you will provide mentorship and support to middle school students (grades 6-8) at Chestnut Middle School, Chestnut TAG, Impact Prep, and Springfield Legacy Academy. You will work closely with a Master's Level Case Manager to help youth and their families build skills, strengthen relationships, and connect with essential community resources. Maintain a caseload of 15-20 youth as assigned by the program Meet weekly with youth as a mentor to support prosocial skill development and healthy decision-making Collaborate with the Master's Level Case Manager on individualized service planning and risk/safety assessments Support youth and families with crisis planning and reassessment as needed Co-facilitate psychoeducational groups focused on social skills and reducing risk of substance use Provide parent education and skill-building sessions at least five times per year Assist youth with academic tutoring and school-based supports Partner with schools, providers, and community resources to ensure coordinated care Support youth and families outside of school hours when needed Attend IEP/504 meetings and contribute to treatment planning Maintain accurate daily progress notes and complete documentation in a timely manner Who You Are Bachelor's Degree in Counseling, Psychology, Social Work, or a related human services field required Experience working directly with youth, families, or individuals with behavioral health or substance use needs Strong communication, mentoring, and interpersonal skills Knowledge of child-serving systems and community resources preferred Cultural competence and ability to work with diverse populations Familiarity with documentation, word processing, and email systems Valid driver's license and reliable transportation required We Hire for Purpose Since 1938, BHN has been dedicated to providing high-quality, affordable, and culturally responsive behavioral health services across Western Massachusetts.
