Job Summary: The Care Manager role involves the employee providing support services for the clients with consideration of dignity and privacy. Support services may include companionship, concierge, medication assistance, personal care services, transportation or other services requested/needed by the client as defined in their service plan.

Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

1.     Complete visits/tasks as scheduled (may be multiple clients in multiple locations).*

2.     Attend required orientation, scheduled in-services and staff meetings.

3.     Comply with all agency policies and procedures, including documentation requirements.*

4.     Maintain client confidentiality.*

5.     Provide services as defined on service plan, which may include companionship, concierge, medication assistance, personal care or transportation.*

6.     Observe and report any safety hazards found in the client’s home, changes in client condition or complaints to the supervisor.*
