Youll work alongside caring professionals who value teamwork, trust, and heart in everything we do.What Youll Do as a Registered Nurse Case ManagerAs a Registered Nurse Case Manager, youll be the cornerstone of our care teamhelping patients live their best quality of life. Each day youll:Coordinate and manage individualized hospice plans of careDeliver hands-on nursing care and emotional support to patients and familiesEducate caregivers and guide them through the hospice journeyCollaborate with physicians, social workers, and chaplains to ensure holistic careRespond promptly to changes in patient needs with expertise and compassionWhat to ExpectThis full-time RN Case Manager role is 40 hours per week, MondayFriday, with a rotating on-call schedule for evenings and weekends. Youll work independently while always supported by a caring team.What Youll BringCurrent Illinois Registered Nurse (RN) licenseGraduate of an accredited nursing program (BSN preferred)At least 1 year of medical/surgical, oncology, or hospice nursing experience (2+ years preferred)CHPN certification preferred (or obtained within 2 years)CPR certification (within 3 months of hire)Strong clinical judgment and ability to work independentlyLocal residence with ability to respond to on-call within 30 minutesPay & Benefits$78,000$108,000 annually, based on experience and educationComprehensive medical, dental & vision insuranceGenerous paid time off that grows with service401(k) with company match (Traditional & Roth)Company-paid life, AD&D, and disability coverageHSA & FSA wellness accountsReady to Make a Difference?If youre an RN who finds purpose in compassion and excellence in hospice care, wed love to meet you.
