In this PRN role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence. Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator. Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures. Performs patient and family instruction on diet, medication, and treatment. Collaborate with the interdisciplinary team to ensure seamless, holistic care What You’ll NeedActive Florida LPN license Applicants must be a graduate of an approved nursing program. Minimum of 1-2 years of experience as a LPN. Home health care experience required. Strongly prefers experience using Homecare Homebase (HCHB) EMR Intravenous (IV) Certification preferred.
