Family Practitioner

POSITION SUMMARY: The Family Practitioner is responsible for providing comprehensive health care and preventive services. The Practitioner diagnoses and treats diseases and injuries of human internal organ systems, examines patient for symptoms of organic or congenital disorders and determines nature and extent of injury or disorder, referring to diagnostic images and test, and using medical instruments and equipment. They are also responsible for prescribing medication and recommends dietary and activity program, as indicated by diagnosis. RESPONSIBILITIES: 1.Evaluation, diagnosis and treatment for patients with acute illness. 2.Preventive care, consisting of periodic physical examination, immunizations, health education and preventive screening. 3.Treatment, counseling, and referral for rehabilitation for maintenance of patients with chronic conditions. 4.Coordination of specialty services for consultation and treatments. 5.Referral and follow-up for patients requiring in-patient care at St. John's Episcopal Hospital/SSD, Queens Hospital Center, Jamaica Hospital or others as required. 6.Will be required to be on telephone "on-call" coverage, for patients. 7.The Family Practitioner will work cooperatively with other allied health providers including the nurses, medical team assistants, counselors, social workers, psychologist, psychiatrist, pharmacist, nutritionist and laboratory technologist in providing comprehensive team health care. 8.The Family Practitioner is expected to participate in Continuing Professional Education programs to provide 50 CME credits per year. 9.The Family Practitioner will be required to participate in other Health Center related tasks as directed by the Medical Director. These may include but not limited to Quality Assurance, Patient Education in the Community, Completion of Survey, etc. 10. Responsible for assuring continuity of care, including patients admitted to the Addabbo's affiliated hospitals. 11.To follow the guides as provided in the Policy and Procedures Manual for family Medicine. Incumbents will follow any other instructions, and perform any other related duties, as may be required by their supervisor MINIMUM QUALIFICATIONS: 1.Graduate of an approved Medical School 2.M.D. Licensure in New York State 3.Board certified or Board eligible in Family Medicine 4.Credential approved by the credentialing Committee 5.BLS Required
