Physical Therapist – Home Health

We are seeking a dedicated Physical Therapist (PT) to join a growing team The ideal candidate will provide comprehensive physical therapy services in a Home Health setting. This role involves conducting evaluations, administering treatments, overseeing discharges, and supervising Physical Therapy Assistants. The PT will create and implement individualized care plans to help patients regain optimal function following injury, illness, or physical impairment. Coordination with assistants is essential to ensure patients achieve their highest level of functional independence.

Key Responsibilities:


  • Conduct thorough evaluations to assess patients' physical abilities and needs.
  • Develop and implement customized treatment plans aimed at restoring maximum function.
  • Perform regular treatment sessions and adjust plans as necessary based on patient progress.
  • Oversee the discharge process, ensuring patients and their families are well-prepared for the next steps in care.
  • Supervise and collaborate with Physical Therapy Assistants, providing guidance and support to achieve therapeutic goals.
  • Maintain detailed documentation of patient progress and treatment outcomes.
  • Communicate effectively with patients, families, and other healthcare team members to ensure coordinated care.

Qualifications:


  • Graduate from an accredited Physical Therapy program
  • Current Physical Therapist license
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
