BAYADA Home Health Care is seeking a RN Case Manager f or our West Chester, PA Pediatrics and Adults home care office . This office provides best in class care to clients throughout West Chester, PA and surrounding areas. Visit our Pediatric/Adult clients and conduct assessments in West Chester, PA and other surrounding areas. Develop the care plan and oversee its execution. Supervise LPN's, RN's, and home health aides (HHA) staff in the field. Facilitate new hire orientation. Conduct competencies on new and tenured staff. Qualifications for a RN Case Manager: Registered Nurse (RN) with 1 or more years of experience as an RN. Supervisory experience preferred.
