Why Amedisys
Amedisys is a leading provider of home health, hospice and high-acuity care, dedicated to helping patients and families navigate the complex healthcare system. Responsibilities
Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.
Completes OASIS assessments timely and accurately.
Provides skilled nursing care to patients in accordance with the p lan of c are and agency policies.
Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
Initiates appropriate referrals when indicated.
P romotes and maintains patient health through teaching and appropriate rehabilitative measures.
Assists patients in learning appropriate self-care techniques.
Other duties as assigned.
Qualifications
Current RN license, specific to the state(s) you are assigned to work.
One year of clinical experience as a n RN . If less than one year clinical experience, you must be approved by VP of c linical.
Current CPR certification.
Valid driver's license, reliable transportation and liability insurance.
