CarePatrol is looking for a Care Transition Advocate in Syracuse, New York to increase revenue and build brand awareness. This role will have a presence in the local community and promote the brand, and use their sales and marketing skills, relationships and experience to drive leads to be converted to sales. They will also be responsible for supporting family meetings and tours to ensure the selection of the best senior living and care options for our clients. They will be a steward of the brand and live our mission to help families and seniors in some of their most crisis situations.


The ideal candidate will want to be both an entrepreneur and part of a team. At CarePatrol, it's all-hands on deck and everyone on the team works towards a common goal--helping seniors and their families. As a Care Transition Advocate, you must have exemplary customer service skills and ability to manage time and workload effectively; excellent written and verbal communication skills; ability to work in a team environment; strong customer service and problem-solving skills; attention to detail; and the ability to engage others. The ideal candidate will also enjoy having a seat at the table of a fast-paced and growing company and bring ideas to make the team and business better. A Care Transition Advocate is a strong sales/marketing individual who is also empathetic and compassionate towards CarePatrol’s clients and families and takes a person-centered approach to finding solutions. If you are someone that likes to sit behind a desk, doesn't like to be in front of people presenting about the business or discussing solutions, or want to help people in crisis situations, this is probably not the role for you.


Some of the day-to-day responsibilities of a Care Transition Advocate include the following:


  • Identifies and develops potential customers and contacts in healthcare, government, business, industry, academia, and/or the service community that present as referral sources and/or agencies/organizations to refer to.
  • Networks with health care providers, social service organizations, long term care providers and other groups in the elder service network.
  • Develop, implement, and maintain quality marketing and outreach strategies to educate potential clients and referral sources
  • Strive to exceed daily, weekly, monthly and year-over-year financial goals and other KPIs.
  • Act as a brand ambassador within the community and with clients demonstrating high level customer service.
  • Lead, motivate, and support the team within a time-sensitive and demanding environment, including development of outreach workflows, marketing strategies and relevant support systems for the team.
  • Partner with all staff to improve the use of proprietary tools and systems to enhance marketing and outreach efforts across the organization.
  • Conducts psychosocial, medical care, and financial intakes and evaluations of seniors to determine needs and establish most appropriate referral or services options.
  • Provides referrals to appropriate resources and agencies to address unique needs of seniors and their families.
  • Supports seniors and their families to engage appropriate resources across all levels of care to meet their specific needs and follows-up at regular intervals to ensure additional resources/referrals are not required.
  • Maintain client records, prepare monthly reports as required.
  • Performs varied outreach duties to inform seniors and their caregivers of programs and services and available resources.
  • Maintain in-depth knowledge of senior living and care options for families.
  • Nurture leads through the sales conversion process.
  • Provide detailed information to clients on the services offered by CarePatrol.
  • Pre-screen inquiries and referrals for viability, track and facilitate progression of all inquiries and referrals to appropriate response such as providing direction to other services, moving to senior care advisor caseload, etc.
  • Visit clients and/or their families in person at hospitals, rehab centers, skilled nursing facilities, and other healthcare facilities to discuss all senior living and care options and help families evaluate all options.
  • Provide appropriate care options for the client based on the care discovery findings. This includes action steps and community resources needed to carry out the assistance required by the client.
  • Schedule and accompany clients and/or their family on various tours to assist families in making final decisions on living and care options.
  • Constantly seek opportunities to increase customer satisfaction.
  • Ensure on-brand customer experience.
  • Schedule regular follow-up with clients and their families to monitor any changes in their care needs.
  • Utilize the company’s CRM system daily to provide accurate and real-time updates on all active clients.
  • Cultivate and nurture relationships with CarePatrol’s senior living partners.
  • Maintain and keep current reporting tools.
  • Obtain testimonials via various review platforms and communicate to team, highlighting for social media posts.
  • Support business development including but not limited to: new senior living community, home-care and affiliate services partnerships.
  • Other responsibilities as assigned.


Desired skills and experience as follows:


  • Bachelor’s Degree preferred.
  • Competitive and driven with a track record of leading sales growth and business development in a healthcare and/or senior living field.
  • Ability to work closely with families to discuss highly confidential medical, financial and other personal matters, and to maintain confidences.
  • Must be willing to reasonably travel within territories as assigned, and requires a valid driver’s license, reliable transportation and insurance coverage.
  • Exceptional interpersonal skills, multi-tasking and problem solving.
  • Professional and dynamic presentation skills.
  • Ability to structure workload and workflow independently.
  • Intellectual curiosity and strong critical thinking skills.
  • Expert in relationship building and proven ability to increase revenue opportunities with new and existing clients and channels.
  • Working knowledge of senior care resources.
  • A thorough understanding of CarePatrol’s brand value, key differentiators and competitive set.
  • Ability to combine sales skills with deep customer empathy to ensure consistent delivery of premium experiences for our clients.


Benefits Package:


The target salary and commission of this position is $80,000.00+. CarePatrol also offers other discretionary bonuses, 3% contribution to company's retirement plan, 15 days of PTO, company holidays, professional development reimbursement, technology, and other benefits.
