JOB PURPOSE:

The Nurse Practitioner Supervisor (NPS) will work with community primary care physicians (CPCPs) to oversee and coordinate participant care alongside the Interdisciplinary Team. The NPS will help manage participants’ functional, clinical, and emotional needs, focusing on their independence and overall wellness. This role connects participants, community physicians, and plan staff to create and follow through on care plans, address changes in participants’ conditions, and ensure preventive care services are delivered.

JOB RESPONSIBILITIES:

  • Manage a PACE site, handling all participants at that location.
  • Supervise Nurse Practitioners and assist with overseeing other clinical staff as needed.
  • Complete performance reviews and evaluations for staff and provide feedback.
  • Act as Medical Director when needed or as directed.
  • Conduct annual wellness assessments for participants, with more frequent assessments for those at higher risk.
  • Provide urgent care and ensure same-day access for participants.
  • Assess and manage acute changes in participants’ conditions promptly.
  • Review participants returning from hospital visits within 5 days, including medication and treatment plan checks, and coordinate follow-up care.
  • Review medical histories every 6 months and maintain accurate diagnostic records.
  • Ensure documentation is complete and accurate, with proper diagnosis codes recorded.
  • Improve clinical documentation practices and provide feedback to staff.
  • Monitor symptoms and conditions to manage care and treatment effectively.
  • Work with PCPs to manage high-risk participants and address any care gaps.
  • Attend and contribute to Interdisciplinary Team (IDT) meetings, and represent CPCPs as needed.
  • Communicate with the IDT and other staff according to policies and procedures.
  • Educate participants, families, and staff on care topics such as disease management and medication adherence.
  • Follow HIPAA regulations and protect patient information.
  • Conduct home visits as required.
  • Supervise the development and delivery of care plans, ensuring they meet individual needs.
  • Oversee home care services provided by the Community Health Network (CHN), including scheduling and ensuring care quality.
  • Provide emergency coverage for primary care providers as needed.
  • Participate in on-call rotation duties.
  • Work within your authorized scope of practice and meet required competencies before working independently.
  • Perform other assigned duties.


Experience:

  • Minimum of two (2) years in clinical nursing practice in-home care, care management, or related field required.
  • Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
  • Effective oral and written communication and interpersonal skills required.
  • High-level analytical and research skills.
  • Ability to manage multiple projects and meet project deadlines.
  • Basic Computer Skills in Microsoft office.

License:

  • Licensed to practice as a Nurse Practitioner in NY required.
  • DEA Certification required.
  • AANP or ANCC Board Certification required.

Additional Requirements:

  • Be legally authorized (for example, currently licensed, registered, or certified if applicable) to practice in the State in which the healthcare professional will perform the function.
  • Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
  • Must have a Medicaid Provider ID and a Medicare Provider ID in good standing
