Title: Occupational Therapist Location: Elk Grove, CA Job Type: FTE/Permanent Position Overview: We are seeking an Occupational Therapist to provide restorative and rehabilitative occupational therapy services to participants both at the facility and in home settings. The Occupational Therapist will work alongside the Primary Care Physician, rehabilitation team, and other interdisciplinary team (IDT) members to maximize participant independence, safety, and function. Essential Job Duties: Conduct Occupational Therapy assessments to determine participants' care needs, preferences, and goals, and actively participate in interdisciplinary team meetings to develop care plans. Deliver and document Occupational Therapy interventions based on physician orders and/or care plans, including the use of devices, techniques, and activities to promote muscle re-education, cognitive function, balance, endurance, and other factors affecting independence in performing Activities of Daily Living (ADLs). Supervise Therapy Assistants, providing training, instruction, and guidance to implement Functional Maintenance Programs (FMP). Provide training and instructions to staff, participants, and caregivers/families in Occupational Therapy-related areas to enhance their interactions with participants. Collaborate with other IDT members to ensure smooth care transitions between different settings (e.g., hospitals, nursing facilities, home) and participate in end-of-life care coordination and support. Job Requirements: Bachelor's Degree in Occupational Therapy from an accredited program. minimum of two (2) years of experience in a clinical setting, preferably with frail or elderly populations. At least two (2) years of supervisory experience, with demonstrated ability to mentor and develop team members (preferred). Registration with the National Board for Certification in Occupational Therapy. Licensed and/or eligible for licensure as required in the state of employment.
