Job Responsibilities: Facilitate open communication between patients and the interdisciplinary team. Establish and maintain therapeutic relationships by offering education and encouraging self-advocacy and independence. Utilize state-of-the-art technology to document the skilled services you provide. Qualifications: Licensed Practical Nurse with a valid CT LPN license. A minimum of one year of LPN experience; homecare experience preferred. A valid driver's license and reliable transportation are required. Computer proficiency is necessary.
