General Summary:

Cabin Creek Health Systems is a Federally Qualified Health Center in southern West Virginia. Our mission is to promote the health and well-being of all people in our communities, especially the most vulnerable, through health care that is guided by science, compassion, and respect and to contribute to the education of skilled and caring health professionals. We are recruiting nurse practitioners and physician assistants for our Advanced Practice Provider (APP) Primary Care Postgraduate Fellowship Program. This is a year-long transition to practice program that provides structured support, precepting, and continuing education for newly graduated nurse practitioners and physician assistants that are interested in pursuing a career in primary care. Our program encompasses precepted continuity clinics, mentorship, didactics, research experience and quality improvement. There is a year-long employment requirement after completing the residency program where salary will be increased to reflect a typical new hire salary with 2 years of experience. Core Duties and Responsibilities, which include the following and other duties as assigned:

APP Fellow will assume responsibility for providing comprehensive primary care services (i.e., health and social histories, physical exams, acute and chronic disease management, formulating differential diagnosis, and providing evidence-based treatments, according to policy and procedures) with supervision from an experienced primary care provider
Participates in weekly specialty clinic rotations at internal and outside locations
Participates in weekly mentorship clinics where APP Fellow will work with an experienced provider seeing patients and consulting on care management decisions in a primary care setting
Collaborates and consults care management decisions with assigned preceptor, as appropriate to the experience and ability of the APP Fellow
Collaborates and consults with fellow allied health professionals to provide comprehensive, individualized patient care
Participates in self-assessment and reflection on a weekly basis
Maintains professional competence through participation in continuing education, at a minimum as required by licensure body
Actively engages in weekly didactic learning sessions with other APP Fellows
Conducts and participates in research and quality improvement initiatives as appropriate and determined by Postgraduate Fellowship Program Director
Participates in research and quality improvement initiatives as directed by Postgraduate Fellowship Program Director
Maintains awareness of trends in primary care and stays up to date with appropriate evidence-based guidelines reflected in practice
Interacts politely and acts as a team player with other providers and staff at the clinic
Participates in evaluations of all aspects of the program on a regular basis, including self-evaluation, evaluation of didactic sessions, evaluation of specialty rotations, and evaluation of preceptors
Works with a Medical Assistant team within the clinic.

Education and Experience

Recent graduate (within 18 months) from an accredited master’s or DNP program

Credentialed as PA-C, FNP-C, FNP-BC, AGPCNP-C or AGPCNP-BC or license/credential eligible prior to the start of the program

Licensed or licensed eligible as a PA-C or APRN

Physical/Environmental

Prolonged periods of sitting, some bending and stretching. Prolonged periods of standing. Lifting and carrying of items weighing up to 25 pounds. Requires manual dexterity to operate office equipment, normal range of hearing and vision.

Travel

Will travel to multiple locations in Kanawha County/greater Charleston area

Working Conditions
