HHA/PCA Long Island

An employee trained to provide, under professional supervision, patient assistance with nutritional and environmental support and personal hygiene, feeding and dressing and/or as extension of self-directed patients, select health related tasks. The PCA reports to the supervising Registered Nurse in charge of the case for patient care and to the Staffing Coordinator/Team Leader for administrative aspects of the job.
QUALIFICATIONS:
Ability to read and write consistent with job requirements.
A successful completion of an approved basic personal care aide-training program consistent with New York State requirements for which a certificate has been obtained, or
b. one full year of experience in providing personal care services through a home care services agency within three years preceding the effective date of an initial license issued pursuant to article 36 of the Public Health Law; or
c. in those instances where the personal care aide is to be providing assistance with health-related tasks, such aide shall be trained, and training in health-related tasks shall be completed in full prior to the personal care aide’s assignment to any patient, as evidenced by written documentation of such completion.
Freedom from health problems that may be injurious to the patient, and provide this via written documentation of such completion.
A documented health status assessment, which is completed prior to the beginning of patient contract and annually thereafter or more frequently if necessary.
A pre-employment physical by a physician of P.A, which is completed prior to the beginning of patient contact, and a documented health status assessment annually thereafter or more frequently is necessary.
The following documented tests and examinations: immunization to rubella as demonstrated by serologic evidence or antibodies or a document indicating one dose of live virus rubella vaccine was administered on or after the age of twelve months, showing the product administered and the date of administration, and prepared by the health practitioner who administered the immunization; or a copy of a document prepared by a physician, physician assistant, specialist assistant, nurse practitioner or a laboratory, which comes from a previous employer or the school which the individual attended as a student;
Test for tuberculosis (TB) are required for employment. Initial Applicants who cannot provide proof of a TB test with negative findings within the last 12 months are required to have a TB test. A positive find shall require appropriate clinical follow up.
Immunization to Rubeola for individuals born after January 1, 1957 or release due to medical contraindication;
Immunity to Hepatitis B or release due to medical contraindication or declination
10. All employees must be up to date with the Covid-19 vaccinations. Only a medical exemption will be accepted as a reason not to be vaccinated.
Any test that may require by the local board of health.
Drug screening prior to employment then every year thereafter.
Employee’s statement declaring he/she has no habituation or addiction to depressants, stimulants, narcotics, alcohol or other drugs.
Background checks prior to employment.
Valid drivers license (preferred).
RESPONSIBILITIES:
Helps patient maintain a clean and hygienic home
Assists in maintaining a healthful, safe environment
Does simple record-keeping, as outlined by nurse
Assists with patient transfer
Positioning and turning of patients who cannot assist workers
Assists patient with bedpan or to the bathroom
Assists patient in Shaving
Assists patient in bathing
Assists patient in dressing
Assists patient in hair grooming
Assists patient in mouth area
Following plan of care
Assists patient in filing of fingernails
Performs general skin care including feet
Use of hydraulic lift
Assists with the use of elastic support stockings
Assists with a condom catheter
Assists with daily catheter care
Assists with the emptying of the urinary drainage bag
Weighs the client
Measures intake and urinary output
Changes sheets with patient in bed
Assists patient with ambulation
Assists patients with guarding, and holding patients who have difficulty ambulating
Active assistance in passive range of motion exercise
Performs light housekeeping tasks, such as:
changing bed linen
dusting and vacuuming client’s room
tidying kitchen, bathroom and bedroom
do shopping if no one else is available
do client’s personal laundry (may include mending or ironing)
Maintains appropriate personal hygiene and dress code
Attends in-service/staff development programs at least 6 hours per year.
EVALUATION: Performance evaluations are done by the BHHC Staffing Coordinator/ Team leader, with input from others in a supervisory and administrative capacity.
