Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.In collaboration with physician, the NP will be providing comprehensive assessment, including multifactorial complexphysical examination and psychosocial needs, self - management education & tools to better manage patient’s multipleconditions. As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primarycare services to an assigned the at risk panel of Elderplan members who reside in the community. The NP will insuredevelopment of care planning and access to preventive care services in accordance with member and Plan health outcomegoals and objectives. The NP will act as clinical resource to the member, family and interdisciplinary team.Masters Degree in Nursing, graduate of accredited Nurse Practitioner program requiredOne year of clinical experience in role of nurse practitioner required; Minimum one year in geriatric specialtyPrevious clinical RN experience in acute, subacute and/or long term care setting preferredPrior experience in managed care a plusAbility to assess, diagnose, treat and follow-upAbility to work independently and as part of interdisciplinary teamsAbility to develop relationships and communicate to members/familyEstablish priorities and maintain schedulesNYS RN/NP License, Registration, DEA certificates
