Physical Therapist (PT) - 13-Week ContractLocation: Silver City, NM Shift: Day Shift Position Overview We are seeking a dedicated Physical Therapist (PT) for a 13-week contract position within a skilled nursing and rehabilitation environment. This role is ideal for a PT experienced in geriatric care, functional mobility improvement, and post-acute rehabilitation. The PT will deliver high-quality therapeutic services to support functional recovery and enhance patient independence. Key Responsibilities Perform comprehensive physical therapy evaluations to determine mobility, strength, and functional needs. Develop and implement individualized treatment plans based on evidence-based practice. Provide therapeutic interventions focused on strength, balance, gait, and functional mobility. Document evaluations, daily notes, and progress reports in alignment with facility and regulatory guidelines. Collaborate with an interdisciplinary care team to ensure coordinated, patient-centered care. Educate patients, caregivers, and facility staff regarding exercises, safety, and functional strategies. Adapt treatment approaches to meet diverse patient needs within a fast-paced post-acute setting. Requirements Active and unencumbered Physical Therapist (PT) license in the state of practice. Experience in skilled nursing, geriatrics, or post-acute rehab strongly preferred. Strong clinical assessment and documentation skills. Ability to work independently and as part of a multidisciplinary team. Must have an active New Mexico Caregivers Account (required by facility).
