All Metro Health Care, a Modivcare Personal Care Service, is looking for a Per Diem RN Supervisor/Instructor. You will have the opportunity to make a positive impact on the lives of our patients by ensuring our caregivers have the skills needed to deliver outstanding care. Pay: $35/Hourly Schedule: 2 days a week. 8a-5p Office: 350 Northern Blvd Suite 206, Albany, NY 12204 & 1462 Erie Blvd. 2nd Floor, Suite #207 Schenectady, NY 12305 You will... Performs on-site supervisory and teaching visits to evaluate caregivers Provides clinical orientation to caregivers on assigned home care cases. Participates in the training of new nurses and in-services for field staff. Performs initial nursing assessments Counsels patient/caregiver in meeting nursing and related needs. Assigns and coordinates services provided. Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs. Prepares clinical progress notes ensuring any changes in the patient's condition is reported Monitors clinical records for complete and accurate documentation. Registered Nurse (RN) 2 years of RN experience 1 year of home care experience required 1 year of direct supervision of Home Health Aides (HHAs) or Personal Care Aides (PCAs) preferred.
