Home Health and Hospice RN every other week on call for Davis and Weber County

Responsibilities:
* Hours required: M-F 5pm-8am, Sat/Sun 8am-8am/Weeks run Friday through the next Thursday
* In home skilled nursing visits, initial and ongoing assessments
* Monitor and report changes in patient symptoms or behavior
* Communicate with collaborating physicians or specialists regarding patient care
* Maintain accurate patient medical records and plan of care
* Provide compassionate emotional support to patients and their family members
* Communicate effectively to interdisciplinary team members
* Observes/ensures compliance with Medicare and State guidelines
Qualifications:
* *Previous experience in home health and hospice care preferred*
* Ability to build rapport with patients, families, and facilities
* *Strong problem solving and critical thinking skills*
* Ability to thrive in a fast-paced environment\*Must be able to pass a background check and drug screen
\*Job Type: Full-time/salary
\*All PPE is provided by the company
