Nurse Practitioner – Family Practice
Salary Range: $125,000 to $140,000 + Sign-On Bonus/Relocation Assistance

Your Role:
As a Nurse Practitioner, you will provide comprehensive patient care in a primary care setting, working under the general supervision of the Chief Medical Officer. Your responsibilities will include:
- Diagnosing, treating, and monitoring acute and chronic conditions following evidence-based guidelines.
- Developing and implementing treatment plans to ensure continuity of care.
- Conducting health maintenance care, physical examinations, and well-baby checks for all ages.
- Collaborating with physicians on complex cases, ordering and interpreting diagnostic tests, prescribing medications, and conducting follow-ups.
- Documenting patient interactions and promoting wellness through health education.
- Participating in community awareness activities and off-site programs to enhance holistic healthcare delivery.
Requirements:
- Current, unrestricted Nurse Practitioner license in Arizona.
- Certification as a Family Nurse Practitioner (FNP).
- Master’s or Doctorate degree in Nursing.
- Minimum of 2 years of experience as a Family Nurse Practitioner, preferably in a primary care setting.
- Excellent clinical and interpersonal skills.
- Strong communication and collaboration abilities.
- Experience in a Federally Qualified Health Center (FQHC) or Primary Health Care environment is preferred.
- Bilingual in Spanish and English is a plus.
