Role Description

This is a full-time On Call Registered Nurse role at Utah Home Health & Hospice. The Registered Nurse will be responsible for providing direct patient care in a home health and hospice setting. The role involves assessing patient needs, developing and implementing care plans, administering medications, collaborating with interdisciplinary team members, and providing emotional support to patients and their families. While this role is primarily located in Utah County.


Qualifications

  • Valid Registered Nurse (RN) license in the state of Utah
  • Prior experience in home health and hospice care
  • Strong clinical skills and ability to assess patient needs
  • Excellent communication and interpersonal skills
  • Ability to collaborate with interdisciplinary team members
  • Knowledge of Medicare and Medicaid regulations
  • Basic computer skills for documentation purposes
  • Compassion, empathy, and the ability to provide emotional support to patients and their families
  • Bilingual skills (Spanish/English) are a plus
