As a Home Health Nurse, you will collaborate with patients, their families, and healthcare providers to create comprehensive care plans tailored to individual needs. This includes determining the frequency and duration of nursing visits, outlining specific treatments, setting achievable goals, and coordinating with other healthcare disciplines.
Your role involves regular home visits to evaluate the patient's health status, manage wound care, and fulfill various nursing duties for individuals with chronic conditions, illnesses, or injuries.
KEY RESPONSIBILITIES:
- Conduct thorough assessments in patients' residences to identify their healthcare needs.
- Formulate care plans in partnership with patients, caregivers, healthcare providers, and case managers, ensuring proper documentation.
- Evaluate the necessity for additional services and equipment upon admission and continuously inform the case manager or supervisor.
- Assess the home environment to recommend enhancements that promote patient safety and care quality.
- Receive and document verbal orders from healthcare providers as required.
- Implement preventive, rehabilitative, and therapeutic interventions, providing guidance to patients and caregivers.
- Maintain professionalism by adhering to high clinical standards and staying updated on healthcare practices.
- Update patient records accurately, including medication profiles and care plans, in a timely manner.
- Serve as a mentor to new staff when necessary.
- Perform additional duties as assigned.
QUALIFICATIONS:
Applicants must possess a degree from an accredited nursing program, hold a current state license as a Registered Nurse, and have CPR certification. A minimum of one year of clinical nursing experience is required, with a preference for home health experience. Candidates must have a valid driver's license, a reliable vehicle, and automobile insurance.
Strong interpersonal and communication skills are essential, along with the ability to conduct home visits and provide skilled nursing assessments and care.
