Summary: DHMF inland Empire is looking for a CV/CT Surgeon to start as soon as credentialed. BID DUE DATE: 11/4 5pm ESTRates24-Hour Call (4 Hours Gratis) CallbackTier 1 (will be reviewed by client first):Callback: $475 and below24-Hour Call (4 Hours Gratis) : $4600 and belowTier 2 (will be reviewed by client after Tier 1):Callback: $476 - $50024-Hour Call (4 Hours Gratis): $4601 - $4700Average call back hours: 7-9.5 hoursSchedule/Availability Requirements: Start: ASAP Duration: 6 months to a year Assignment: Clinic is Monday-Friday 8a-5p. Additionally, 4 to 7 consecutive 24hr call days per month. Facility Location: Hospital Coverage- St Bernadine; 2101 N Waterman Ave, San Bernardino, CA 92404Clinic-401 E Highland Ave, San Bernardino, CA 92404 (located right behind the hospital) Setting: Clinic and Hospital FTE: 1 Providers Needed: 1 Duties: inpatient consults, outpatient/office consults, follow up appointments, surgeries, call coverage Required Procedures: CABG, valve surgery, TAVR, lung/pleura surgery EMR: Cerner Patients per Day: Varies 2-4 a day Support Staff: 4 APPs and 2 other surgeons Reason for Coverage: Physician on medical leave Deal Breakers? Unreliability or tendency for last minute cancellation Travel Preferences: Preferred Lodging: Hampton Inn San Bernardino or nearby the hospital (within 15mins of hospital) Air Travel: Local, no flights HIGHLY PREFERRED Credentialing Information: Timeframe: 90 days Requirements: Emphasize to candidate that credentialing takes at least (90) once a completed packet is received DHMF packet required; St. Bernadine packet required if practice includes inpatient work &/or Hospital credentialing Credentialing with other facilities? St Bernardines Hospital and DHMF Billing enrollment: Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? No Additional Details / Questions Answered by Facility: Is the facility looking for the provider to cover general surgery, trauma, and ICU? ICU only for the patients they operate on. No to general surgery or trauma. Is the provider expected to round on their patients daily? Yes, but the surgeon would only round on the days they are on call. Is the 24-hr call in house or beeper? No, we call the surgeons cell phone Are there a peds and burn surgeons, or would that be the responsibility of the trauma surgeon? Responsibility of the ED or Trauma. CT surgeons are only called for consults by cardiologists. Would you consider a candidate that is BC in General Surgery but did a CT Surgery fellowship?
