Your Role as Director of Case Management Services Manage the daily operations of the Case Management department, focusing on human resource management. Oversee the interdisciplinary care plan and discharge planning process, ensuring quality patient care outcomes and effective services. Act as a strong advocate for patients and families, ensuring services align with their needs. Provide mentorship and support to case managers and staff, ensuring efficient caseload management and compliance with regulations and policies. Foster relationships with insurance companies, self-insured employers, and other healthcare networks to enhance service delivery. Celebrate employee successes and contributions to the organization. Qualifications CCM® or ACMTM certification preferred. Must be qualified to conduct assessments independently within professional discipline scope. If applicable, possess an active license for the discipline in the hospital's state. Nurses must hold a BSN and current RN licensure. Other professionals should have at least a bachelor's degree; a graduate degree is preferred. Minimum of three years of case management experience in a hospital setting, including experience in Utilization Review and Discharge Planning. Willingness to work various shifts, including weekdays, weekends, and holidays as needed.
